How to Refer to a Dietitian

For care home residents

To refer to the Ipswich Hospital Nutrition and Dietetic Service, you will need to send a completed MUST referral form along with a three-day food diary and weight history. Please provide as much information as possible, and ensure that all sections are complete. If you are unsure of how to work out a patient's MUST score please see the additional information on the Malnutrition Universal Screening Tool (MUST) page of this website.

Please note that inappropriate and incomplete referrals will be sent back to the referrer.

For patients in their own home

If you are a GP or healthcare professional wanting to refer a patient who lives in their own home, you will need to send a completed referral form. Please provide as much detail as possible, including a weight history, past medical history and current medication, and ensure all sections are complete. 

Please note that inappropriate and incomplete referrals will be sent back to the referrer.
