Adult Community Dietetic Services

Our experienced community team supports adults in a variety of settings, including their own home, sheltered accommodation, supported living, care homes, hospices and community hospitals.

We provide patient centered dietetic care plans for people with a range of conditions including malnutrition, enteral feeding and therapeutic diets.

Community hospitals

Dietitians regularly visit the following community hospitals:

  • Blue Bird Lodge Community Hospital;
  • Felixstowe Community Hospital; and
  • Aldeburgh Community Hospital.

Patients are nutritionally screened upon admission using Malnutrition Universal Screening Tool (MUST) and referred to the dietitian if they are at high risk of malnutrition or require advice on therapeutic diets. Staff follow an action plan which promotes nutritional intake and enables them to refer to a dietitian if appropriate.

Home visits

Home visits are available for housebound patients who are unable to attend an outpatient appointment.

Care homes

We work with residents in approximately 85 care homes across East Suffolk.

We have a robust Food First pathway for residents in care homes, and routinely provide study days for care home staff to ensure up to date, evidence based guidance is implemented. Residents are screened regularly for malnutrition using Malnutrition Universal Screening Tool ‘MUST’, and commenced on the food first pathway if at medium risk of malnutrition. Staff are able to directly refer to our department if residents are at high risk of malnutrition and need dietetic input.
