Viewing Your Own Medical Records

Under the Data Protection Act 1998 you are entitled to view your own medical records.  We have produced a guide which explains what you can expect, and contains an application form that you should fill in if you wish to see your records.

To ask to see your own health records (a Subject Access Request) you will need to fill in an Application for Access to Medical Records Form (see below). 

If you are unable to print off a form, copies can be obtained from the Patient Advice and Liaison Service located near South Reception (Entrance 2). They can also be contacted on 01473 704781 or Freephone 0800 328 7624.
 
Who can see my records?

  • You.
  • A person authorised in writing by you on your behalf.
  • A parent or guardian of a child under 16.
  • Any person appointed by a court authorised in writing.
  • A representative of a deceased patient authorised in writing.

What happens next?

  • The Access to Health Records team will process your request and contact you if they need further information.
  • There is normally a charge for this service to cover administration costs up to £50 – this will be requested in advance.
  • The Access to Health Records team will comply with the request within 40 days of receipt of the fee.
